Transitional Support
Transitional support is often needed when young people are due to leave children’s care services and are about to enter the adult world of care. We work closely with the young people, their families and carers to provide the necessary support when leaving home or a residential college. Change can be stressful, especially for those with learning disabilities and autism, but good planning that involves the young person helps to reduce anxieties and eases the path to a new way of life.
All young people benefit from routine and a structured environment but when that secure world is seemingly threatened, it can cause great stress and anxiety. To reduce the trauma that can be associated with changing homes, we work with young people and their families or carers to ensure that the transitional phase is an effective bridge from being a child to becoming more independent and responsible.
